I, the undersigned, would like to confirm my membership in the Teaching and Research Faculty Organization at Tel Aviv University (hereinafter, "the Organization"). I confirm that the Organization's
statute and goals are known to me, and agree to comply with regulations and terms of the Organization's
statute which may change from time to time by the Organization's General Assembly and its executive committee. I confirm the accuracy of the details I have entered on this form and I undertake to inform the Organization of any changes that will apply to it. I am aware that membership in the Organization entails payment of membership fees at a rate set from time to time by the organization's General Assembly, which are deducted from my salary. I am also aware that even if I do not confirm my membership in the organization, organizational handling fees will still be deducted from my salary, as they are automatically collected from all members in accordance with the law, but I will not be entitled to the various rights that membership in the organization confers, including but not limited to, a holiday gift. I authorize the University to give the Organization information on the amount of membership fees the University transfers on my behalf every month.
